A utilização do sistema de bales no estudo da interação enfermeiro-paciente

Crescente número de autores reconhece a centralidade das interações verbais na prestação da assistência de enfermagem. Verifica-se na literatura especifica da nossa profissão uma diversificação no uso de instrumentos ou sistemas de analise de interações. Descreve-se neste trabalho o uso de um sistema de análise de interação no contexto da enfermagem hospitalar: O Interaction Process Analysis, de Bales. O objetivo do estudo foi investigar a existência de um padrão de interação entre o grupo da enfermagem e pacientes. Utilizou-se a técnica de observação direta do comportamento resultante da interação entre os componentes do grupo da enfermagem e pacientes recém-admitidos em unidades de interação. Os sujeitos de interação foram observados durante dez períodos de quatro horas e seus episódios verbais foram registrados em formulários pelos observadores. As interações verbais oriundas do registro foram classificadas à luz do Sistema de Bales e agrupadas nas áreas positiva, neutra e negativa. Foi constatado preponderância de interações da área neutra (85,4%) sobre as demais: positiva (8,4%) e negativa (6,1%), confirmando a existência de um padrão no comportamento de interação entre os sujeitos estudados e demonstrando que a interação entre o grupo de enfermagem e os pacientes se dá em torno de problemas funcionais. A interação ocorre na medida do desempenho do papel instrumental dos integrantes do grupo profissional estudado. O sistema de análise de Bales mostrou grande utilidade na detecção de padrões de interação do grupo da enfermagem com pacientes hospitalizados.
A growing number of investigators is recognizing the central role of verbal interaction in nursing assistance. A diversity of tools or systems of interaction analysis has been detected in the specific literature of our profession. In the present study we describe the use of an interaction analysis system within the context of hospital nursing, i. e., the interaction Process Analysis of Bales. The objective was to investigate the existence of a pattern of interaction between the nursing team and the patients. The technique used was direct observation of the behavior resulting from interaction between members of the nursing team and patients newly admitted to the wards. The interacting subjects were observed for 10 periods of four hours each and six verbal episodes were recorded on forms by the observers. The interactions obtained from the record were classified on the basis of the Bales System and grouped into positive, neutral and negative areas. There was a preponderance of the neutral area (85,4%) over the positive (8,4%) and negative (6,1%) areas, confirming the existence of a pattern of interaction between the subjects studied and demonstrating that the interaction between nursing team and patients is centered around functional problems. The interaction Bales analysis system proved to be very useful in the detection of interaction patterns between the nursing and hospitalized patients.
