A interação enfermeiro-usuário no contexto da reforma psiquiátrica: aspectos da comunicação não verbal

Compreendendo que a área de assistência de saúde mental passa por uma reestruturação, este estudo procurou uma aproximação desta realidade pela análise da interação enfermeiro-usuário no contexto da reforma psiquiátrica, observando os aspectos da comunicação não-verbal inseridos neste contexto. Julgamos ser a forma não-verbal de se comunicar a que mais adequadamente pode evidenciar uma relação interpessoal como de fato ela ocorre, pois envolve expressão de sentimentos, gestos, espaço físico, local, toque, entonação de voz, entre outros.
Nossos objetivs foram:
verificar o conhecimento do enfermeiro que atua em assistência psiquiátrica a respeito do processo da reforma psiquiátrica; verificar a percepção da influência do processo da reforma psiquiátrica em sua prática de assistência diária; e observar se a assistência feita pelo enfermeiro ao usuário de saúde mental reflete, de forma não-verbal, os preceitos da reforma psiquiátrica. Desenvolvemos nosso estudo em 11 instituições públicas, incluindo hospitais fechados, unidades em hospital geral, hospitais-dia, centro de atenção psicossocial e ambulatório instalados no município de São Paulo, que prestavam, exclusivamente, assistência psiquiátrica, observando e registrando a interação entre o enfermeiro-usuário, o contexto e as características de funcionamento de cada unidade. A análise dos dados permite-nos afirmar que os sujeitos pesquisados conhecem alguns dos aspectos da reforma psiquiátrica, porém de modoinsuficiente e, às vezes, de maneira distorcida. Todos enfermeiros responderam afirmativamente quanto à influência da reforma em sua prática, mas, apesar de reconhecermos que há esforços, a análise da realidade e contexto das instituições mostraram-nos que ainda existem muitas contradições, mesmo em equipamentos de assistência considerados alternativos e modernos, levando-nos a inferir que a referida influência parece acontecer mais em seus discursos do que ) efetivamente em suas práticas. Os aspectos não-verbais observados tanto nas interações ocorridas entre os enfermeiros e usuários, quanto no contexto que os envolvia, evidenciaram que muitos dos preceeitos da reforma psiquiátrica não são contemplados. Como exemplo, citamos o uso do espaço físico, portas trancadas, acesso negado a pátios, controle do uso de telefones e banheiros, os locais nem sempre adequados onde ocorreram as interações observadas, o tempo utilizado nestas interações, entre outros. Em nossa percepção, a maioria dos enfermeiros pesquisados não está atenta o suficiente para perceber conscientemente o quanto a forma não-verbal de assistência pode dificultar o que consideramos a principal tônica da reforma psiquiátrica, a humanização.
Understanding the issue of mental health care is passing through a restoration, this search tried to reach a reality analyzing the usual nurse interaction inside of the psychiatry contest of this restoration, observing the aspects of non verbal communication insert in this issue. We judged to be the non verbal communication the properly way of the relation of themselves as the fact as occur, because involve the expression, the feeling, the gesture, the space, the local, the touch, the voice intonation and others. Our point of view were know how was the nurse's knowledge which works in psychiatry care in this procedure of the psychiatry restoration, verify the perception of the influence from the psychiatry procedure's restoration that has done in the daily assistance and observe if the nurse's attendance to the user mental health care reflect in non verbal the precept of the psychiatry reformed. We developed our studies in 11 public institutions, including psychiatry's hospitals, general's hospitals, day hospital, care center psychosocial and consulting-room installed in São Paulo municipality which give exclusive psychiatry's assistance, observing and registering, the usual nurse's interaction themselves, the context and the each unit function characteristics. The datum analysis allowed us to confirm that the people searched have known a few aspects of the psychiatry restoration, however the insufficient way and some times distort. All nurses answered affirmative aboutthe reformed influence in practice, but we recognize the effort, the reality's analyze and context of the institution showed that still many contravention, even between the assistance equipment that are considerate alternative and modern, took us to considerate the mentioned influence likes happen more in their speech them in fact are in their practice. The non verbal aspects observed as much ocurred interactions between usual nurses as the ) contest that involved; this evidence many precept of the psychiatry's reformed are not reached. As an example we mention the space; looked doors, access refused to the yard, controlled of the telephones and toilets, the local is not always adequate where occur the interaction's observation, the time spend and this section and others. Our perception say that most of the nurses searched are not enough attentive to realize consciously and how much non verbal way of assistance could difficult what we considerate the main topic of the psychiatry's reformed, the humanization
