Translation to portuguese and validation of a severity scoring instrument in ICUTISS-28 - Therapeutic Intervention Scoring System

Este estudo teve como objetivos traduzir para o português e avaliar a confiabilidade do Therapeutic Intervention Scoring System de 28 itens (TISS-28), sistema de classificação da gravidade do doente grave, medida pelas intervenções terapêuticasrealizadas. A finalidade principal do instrumento, atualmente, é dimensionar a carga trabalho de enfermagem em UTI. O estudo foi delimitado na análise da confibialidade do instrumento, medida pela repetibilidade entre dois observadores e daconsciência interna dos ítens das categorias que compõem o instrumento. O TISS-28 é um instrumento que contém sete áreas temáticas ou categorias que reúnem intervenções pontuadas de acordo com as intervenções realizadas frente às condiçõesclínicas do paciente avaliado. Após o processo de tradução para a língua portuguesa e adaptação do texto traduzido, foi realizada sua aplicação em 200 pacientes adultos, internados em UTIs diversas, a fim de se avaliar a consistência interna doinstrumento. A coleta de dados para análise da reperibilidade entre dois observadores foi realizada pela pesquisadora e por uma segunda enfermeira com 38 pacientes iniciais da amostragem. Os resultados da análise foi feita por meio do teste deKappa para a mensuração da repetibilidade, demonstraram uma concordância média entre os avaliadores de 92,59% e Kappa médio de 0,80. A análise comprovou a necessidade de revisão técnica dos ítens A3, medicação única, e C4, cateter arterialperiférico quealcançaram Kappa médio de 0,47 e 0,28, respectivamente. A análise de consistência interna dos ítens, por meio do coeficiente Alpha de Cronbach revelou que o TISS-28 apresenta 28 medidas independentes que não comportam consolidaçãoou redução. Apesar disso, os resultados são indicativos de que o TISS-28 pode ser aplicado em nosso meio com alto índice de concordância, devendo ser, acompanhado de um manual de definições operacionais.
The present study aimed to translate to portuguese and to assess the reliability of the 28-Items Therapeutic Intervention Scoring System (TISS-28), a severity classification system of the critical patient, measured by the therapeuticinterventions performed. Currently, the instrument´s main objective is to dimension nursing workload in ICU. The study was delimited on instrument reability analysis, measured by the reproducibility between two observes, and by the analysis ofitems internal consistency. The TISS-28 is an instrument that comprises seven thematic headings or categories, that group interventions scored by the interventions performed according to the evaluated patient´s clinical conditions. After theprocess of translation to portuguese and validation of the translated text, it was applied in 200 adult patients from different ICUs, in order to assess the instrument´s internal consistency. For the analysis of the reproducibility between twoobservers, data collection was conducted by the author and another nurse, and included the first 38 patients from the sample. That analysis used Kappa´s test to measure the reproducibility and the results demonstrated a mean interobserveragreement of 92,59% and a mean Kappa of 0,80. The analysis proved the need of technical review of the items A3, single medication, and C4, peripheral arterial catheter, which achieved a mean Kappa of 0,47 and 0,28, respectively. Internalconsistency items analysis, calculated by Cronbach´sAlfa Coefficient showed that the TISS-28 comprises 28 independent measures that do not permit consolidation or reduction. Despite that, the results indicate that the TISS-28 may be appliedinour reality, with a high degree of agreement, and must be followed by an operational definitions workbook.
