Enfermeiras que cuidam de mulheres: conhecendo a prática sob o olhar de gênero

O estudo foi motivado por pesquisa anterior em que enfermeiras atuam nos cuidados diretos à saúde da mulher, mas referem-se a um poder médico limitante da sua autonomia profissional. Nesse espaço, as mulheres mantêm-se sob uma ordem que lhes nega como sujeitos capazes de decidir politicamente sobre o seu corpo e sobre a sua vida. Parte do pressuposto de que naquela realidade, soma-se ao poder médico a incorporação pelas enfermeiras de um modelo de assistir em saúde vinculado às estruturas de poder do Estado, essencialmente androcêntricas, reproduzindo-o na relação com a cliente. A pesquisa teve como objetivos conhecer os condicionantes institucionais da prática das enfermeiras nos cuidados à saúde da mulher e com base nestes como se constrói e como se dá a relação enfermeira-cliente analisando-a segundo a abordagem de gênero. Os dados foram obtidos por meio de oficinas de reflexão e a análise mostrou que a relação enfermeira-cliente se constrói e se dá em meio a desigualdades entre e intragênero e a prática está condicionada a questões estruturais e à dinâmica do serviço em que médicos definem rotinas e prioridades. A prática das enfermeiras é orientada pelo referente masculino que as inferioriza no campo do saber e a idealização do cuidar define uma relação com as clientes que dificulta a potencialização da capacidade reivindicatória das mulheres e dessas profissionais por direitos. As enfermeiras buscam o exercício do cuidar como expressão de um saber qualificado que conjugue o técnico e o científico com a afetividade, o que é comprometido pelo referencial da prática e pela ordem institucional
This study encouraged by a previous study in which nurses give assistance to women health but refer to a limiting medical care of their professional autonomy. In this environment, women are under an arrangement which denies them as subjects able to politically decide on their body life. It is based on the assumption that in this reality the medical power is added to the nurse's incorporation to assist health associated to structures of the State authority, essentially androcentered, reproducing it in their relationships with the patients. The objectives of this study are to acknowledge the institutional conditions of the nurse's practice in the assistance to women health and based on it, to learn how to construct and how the patient-nurse relationship occurs, analysing it accordingly to the gender approach. Data were obtained by means of reflection workshops and the analysis showed that the nurse-patient relationship is constructed and it is performed in the middle of inequalities in between and intragender and the practice is conditioned to structural questions and to dynamics of the service where doctors define their routines and priorities. The nurses' practice is male referenced which put them in an inferior position regarding the knowledge field and the idealization of caring defines a relationship with the patients which make difficult the claiming capacity of women and the professionals to develop their rights. Nurses aim to perform assistance as an expression of qualified knowledge which could match the technical and scientific with affection, involved by the referential of the practice and by the institutional pattern
