Os lares abrigados: um estudo de caso da situação atual de moradores de um hospital psiquiátrico

O estudo tem como objetivo conhecer a situação atual dos moradores de hospitais psiquiátricos com base no desenvolvimento da Reforma Psiquiátrica no Brasil, cujas políticas específicas vêm sendo propostas para esta população. Como objetivos específicos, buscou-se caracterizar os locais de moradia, descrever e analisar o dia-a-dia vivido pelos moradores à luz da reabilitação no contexto da desinstitucionalização e apreender como esta instituição conhece seus moradores. Para tanto, foi realizado um estudo de caso dos moradores dos lares abrigados do Hospital Psiquiátrico Pinel, eleito em razão do número significativo de moradores, cerca de 60 e pela localização em uma região do Município de São Paulo com grande número de leitos psiquiátricos disponíveis e sua história de instituição psiquiátrica pública que vem presenciando a implantação da Reforma Psiquiátrica em São Paulo. Distintas fontes de dados foram utilizadas para este estudo, por causa de sua complexidade como: a observação simples, dados dos prontuários dos moradores, observação direcionada das casas dos lares abrigados, entrevistas com profissionais e os moradores desses lares. Os dados foram submetidos à análise de conteúdo temática. Pode-se conhecer o cenário, no qual os lares são caracterizados como casas, cuja estrutura física encontra-se suficientemente bem mantida, mas, onde os moradores pouco conseguem tomar parte das decisões de seu funcionamento. Em sua maior parte, eles são pessoas com --------------------------------------------|a longa história institucional, alguns vindos de outras instituições psiquiátricas ou não e estão nos lares abrigados pelas condições de manutenção do autocuidado e o cuidado das casas. Ao se olhar nesse cenário seus personagens e suas atuações, ou seja, o dia-a-dia vivenciado por esses moradores depara-se com duas categorias. A primeira Real X Ideal - A Instituição e o Serviço, apresenta dois polos: de um lado a idealização da reabilitação e de outro a )realidade das relações institucionais no dia-a-dia dos moradores e profissionais que trabalham com eles. Na segunda categoria, O morador: habitando ou mantendo é discutida a presença marcante de práticas ergoterápicas no dia-a-dia desses moradores, assim como suas possibilidades contratuais em seu espaço de moradia. Desse modo, o morador dos lares abrigados é apresentado sob a perspectiva dos profissionais e dos próprios moradores, como um sujeito com diversas necessidades, sonhos e ações que nem sempre encontra lugar no dia-a-dia institucional. Na realidade do Hospital Psiquiátrico Pinel, constitui-se como um último detalhe. Permanece, assim, a instituição psiquiátrica isolada do território onde está localizada, com poucas possibilidades de acesso aos recursos exteriores, bem como dos avanços conquistados na assistência em saúde mental pela Reforma Psiquiátrica no Município de São Paulo.
The study aims to know the present situation of the Psychiatric Hospitals' residents, based on the development of the Psychiatric Reform in Brazil, where specific politics have been proposing for this population. As specific objectives, the characterization of the housing location was sought, describe and analyze the residents' day-by-day, guided by the rehabilitation on the deinstitutionalization and learn how this institution knows their residents. To achieve these goals, a case study about the sheltered homes' residents of the Psychiatric Hospital Pinel was made, chose because it's significant number of residents, around 60, and because it's location on the surroundings of Sao Paulo district with a lot of available beds and it's history as a public psychiatric institution, that have been witnessing the implantation of Psychiatric Reform in Sao Paulo. Different sources of data where used for this study, because it's complexity, like: the simple observation, residents' records data, observation focused on the sheltered homes' houses, interviews with professional and residents of this homes. The data were submitted to thematic content analysis. The scenery can be known, where the homes are characterized as houses, whose structure is sufficiently supported, but the residents almost don't take part of its operation. The majority of this people, with a long institutional history, some came from other institutions psychiatrics or not and they are on the sheltered homes because they have conditions to self-care and housekeeping. Looking at these sceneries, it's characters and their actions, in other words, the day-by-day lived for these residents faces two different categories: The first category Ideal X Real - The Institution and the Service, presents two sides: first, the idealization of rehabilitation and second the reality of institutional relations on the day-by-day of the residents en the professionals which words with them. The second category, The resident: qualified or keeping, the ergotherapic practices on the day-by-day of the residents is discussed, like his contractual possibilities in his home' space. At this way, the resident of the sheltered home is presented under the perspective of the professionals and the residents themselves, like someone with a lot of necessities, dreams and actions, which not always find it's place on the institutional day-by day on the reality of the Psychiatric Hospital Pinel establishes as the last issue. This way, the psychiatric institution keeps isolated where it's located, with low access to the outside resources and also from the advances purchased on the mental health assistance through the Psychiatric Reform of Sao Paulo district.
