Consulta de Enfermagem como proposta de atendimento no Centro de Atenção Psicossocial infanto-juvenil

Os Centros de Atenção Psicossocial infanto-juvenil (CAPSi) são cenários recentes ao suporte imperativo e progressivo da ascendente demanda à criança/adolescente em sofrimento psíquico e família, no qual o enfermeiro encontra-se inserido em equipe interdisciplinar, com o desafio de implicar bases teóricas e assistenciais, que ofertem subsídios a ampliação da sua atuação no campo psicossocial. Deste modo, o objetivo desta pesquisa foi construir e aplicar em prática assistencial um Modelo de Consulta de Enfermagem à criança/adolescente e família atendidos em CAPSi, ao pressupor que essa é uma estratégia de reconfiguração dos saberes e práticas do enfermeiro, que promove o reconhecimento do seu papel profissional na saúde mental e contribuir para o fortalecimento das propostas psicossociais em consonância com os preceitos da Reforma Psiquiátrica. Do ponto de vista teórico, foi utilizado o referencial do Tidal Model, elaborado por Phil Barker e Buchanan-Barker. Em congruência, o enfoque metodológico foi a abordagem qualitativa, delineada pela Pesquisa Convergente Assistencial (PCA). O campo da pesquisa foram dois CAPSi da cidade de Fortaleza-CE. Os participantes foram as enfermeiras que atuavam há quatro meses ou mais nos CAPSi, sendo envolvidos na convergência do estudo, por meio do preenchimento de formulário e diálogos, direcionados por entrevista semiestruturada e encontros educativos. Os dados foram submetidos à técnica de análise de conteúdo temático, sendo compreendidas, ao dar voz às enfermeiras, o transcorrer de vivências cotidianas de atividades em CAPSi, trazendo os caminhos para interdisciplinariedade nesses serviços de saúde mental. Os resultados evidenciam a experiência das enfermeiras com o trabalho em equipe, em núcleo de convergência de conhecimento, com o desvelar do “Navegar” da enfermeira e seu papel profissional, com as estratégias utilizadas por elas no atendimento em CAPSi, com inclusão da família e atuação na construção do Projeto Terapêutico Singular, fomentando a discussão e a reflexão sobre o arsenal de saberes que embasam sua prática na saúde mental infanto- juvenil, compondo sete subcategorias. Convergindo à valorização do kit de ferramentas das enfermeiras, foram expostos limites e possibilidades à Consulta de Enfermagem; dando suporte a etapa de construção do instrumento com base em Tidal Model e avaliação da exequibilidade deste. Por fim, foi possível confirmar a tese, ao compreender que convergir saberes e práticas, enfermeiras e pesquisadora modificaram o âmbito assistencial, com a inclusão de um referencial teórico e aplicabilidade de uma atividade privativa do enfermeiro, a Consulta de Enfermagem, a qual facilitou o reconhecimento da profissão na saúde mental em interface ao fortalecimento do modo psicossocial.
The Child and Adolescent Psychosocial Care Centers (CAPSi) are recent scenarios for the progressive and progressive support of the ascending demand for the child / adolescent in psychological distress and in which the nurse is inserted in an interdisciplinary team, with the challenge of involving theoretical and assistance bases, which offer subsidies to expand its activities in the social and political arena. Thus, the objective of this research was to construct and apply a Nursing Consultation Model to the child / adolescent and family assisted in CAPSi, as it is a strategy of reconfiguration of nurses’ knowledge and practices, which promotes recognition of their role professional development in mental health and contribute to the reinforcement of psychosocial proposals in line with the precepts of the Psychiatric Reform. From the theoretical point of view, the Tidal Model framework, developed by PhilBarker and Buchanan-Barker, was used. Consistently, the methodological approach was the qualitative approach, outlined by the Convergent Care Research (PCA). The field of research was two CAPSida city of Fortaleza-CE. The participants were nurses who had been working for four months or more in the CAPSi, being involved in the convergence of the study, through the filling of the form and dialogues, guided by semi- structured interview and educational meetings. The data were submitted to the thematic content analysis technique, being understood, when giving voice to the workers, the daily experiences of activities in CAPSi, bringing the paths for interdisciplinary in these mental health services. The results show the experienced nurses with the teamwork, in a core of convergence of knowledge, with the "Navegar" of the nurse and their professional role, with the strategies used by them in the care in CAPSi, with inclusion of the family and performance in the construction of the Somatic Therapeutic Project, fomenting the discussion and the reflection on the arsenal of knowledge that bases its practice in the infantile-juvenile mental health, composing seven subcategories. Converging to the valorization of the nurses’ toolkit, the limits and possibilities were exposed to the Nursing Consultation, supporting the construction phase of the instrument based on Tidal Model and evaluating its feasibility. Finally, it was possible to confirm the thesis, by understanding that converging epistemic knowledge, nurses and researcher modified the scope of care, with the inclusion of a theoretical reference and applicability of a private activity of the nurse, the Nursing Consultation, which facilitated the recognition of the profession in mental health at the interface to strengthening the psychosocial way.
