Extubação orotraqueal eletiva: uniformizar cuidados de enfermagem em cuidados intensivos

A Extubação Orotraqueal Eletiva consiste no conjunto de procedimentos que garantem a remoção programada do tubo orotraqueal da via aérea. É uma etapa importante na recuperação do doente, embora não isenta de riscos de complicações ou falhas. Neste sentido, os instrumentos de uniformização de Cuidados de Enfermagem permitem que a Extubação Orotraqueal seja executada tendo em conta os riscos, visando a segurança da pessoa em situação crítica, a qualidade dos cuidados, e os importantes benefícios para os serviços e instituições. No decurso do Estágio Final, foi desenvolvido um Projeto de Intervenção na Unidade de Cuidados Intensivos, que promovesse benefícios, quer para o utente, quer para a instituição, através da implementação de uma ferramenta, baseada na evidência científica mais atual, que permitisse uniformizar os cuidados de enfermagem na Extubação Orotraqueal Eletiva, garantindo a segurança do utente e a melhoria dos cuidados de saúde. Em simultâneo, procedeu-se à análise reflexiva do processo de aquisição e desenvolvimento de competências de Enfermeiro Especialista em Enfermagem Médico-Cirúrgica: Pessoa em Situação Crítica, e de Mestre em Enfermagem.
Elective Orotracheal Extubation consists of a set of procedures that ensure the programmed removal of the orotracheal tube from the airway. It is an important step in the patient's recovery, although not free from the risk of complications or failures. Concerning this, instruments for uni- formity of procedures allow the Orotracheal Extubation to be performed taking into account these risks, aiming at the safety of the patient in critical situation, the quality of the nursing care, with important benefits for services and institutions. During the Final Clinical Practice, an Intervention Project in the Intensive Care Unit was devel- oped, which promoted benefits both for the user and for the institution, through the implementation of a protocol, based on the most current scientific evidence, that would allow the standardization of nursing care in the Elective Orotracheal Extubation, ensuring the safety of the user and the improve- ment of health care. At the same time, a reflexive analysis was made of the process of acquisition and development of competencies of Nurse Specialist in Medical-Surgical Nursing: Critical Patient, and Nursing’s Master was performed.
