O enfermeiro na linha da frente: avaliação e intervenção no risco de maus tratos à criança

Os maus tratos na criança e no jovem assumem atualmente uma grave problemática social, que é importante acompanhar, compreender e resolver, através da sua deteção e do seu diagnóstico precoce. É uma área de atuação particular do Enfermeiro Especialista em Enfermagem de Saúde Infantil e Pediatria (EEESIP), transversal a todos os contextos da prestação de cuidados em que este tem um papel crucial de primeira linha. A metodologia utilizada para desenvolver competências de EEESIP no cuidado às crianças e famílias na maximização da sua saúde, nomeadamente na avaliação e intervenção no risco de maus tratos à criança, e para contribuir para a adequada avaliação e intervenção no risco de maus tratos à criança/ao jovem e família pela equipa de enfermagem foi a realização de estágio em diferentes contextos e a análise sobre a prática de cuidados, sustentadas na evidência científica, e orientadas pelos Modelo de Sistemas de Betty Neuman e os quadros de referência da profissão e daárea de especialidade. A reflexão e análise das práticas de enfermagem relativas à prevenção e intervenção de situações de maus tratos às crianças permitiram compreender o papel do enfermeiro nos Núcleos (Hospitalares) de Apoio a Crianças e Jovens em Risco (N(H)ACJR) e as limitações existentes em efetivar uma intervenção que se encontra legalmente bem definida, patenteadas pelo deficit, sobretudo no preenchimento da avaliação do risco familiar preconizada no Programa Nacional de Saúde de Infantil e Juvenil (PNSIJ). Para a obtenção de ganhos em saúde nesta área, prevenindo a ocorrência/recorrência de situações e evitando o perigo, é fundamental investir na identificação das crianças em risco e do meio que as rodeia e na formação e sensibilização dos enfermeiros.
Abuse of children and young people is currently a serious social problem, which is important to monitor, understand and resolve, through early detection and diagnosis. It is a particular area of activity of the Nurse Specialist in Child Health and Pediatrics Nursing (EEESIP), transversal to all contexts of care provision in which they have a crucial first-line role. The methodology used to develop EEESIP skills in caring for children and families in maximizing their health, namely in assessing and intervening in the risk of child maltreatment, and to contribute to the appropriate assessment and intervention in the risk of child maltreatment/ to the young person and family by the nursing team was the carrying out of an internship in different contexts and the analysis of the practice of care, supported by scientific evidence, and guided by Betty Neuman's Systems Model and the reference frameworks of the profession and area of specialty . Reflection and analysis of nursing practices related to the prevention and intervention of situations of child abuse allowed us to understand the role of nurses in the (Hospital) Support Centers for Children and Young People at Risk (N(H)ACJR) and the existing limitations in carrying out an intervention that is legally well defined, patented by the deficit, especially in completing the family risk assessment recommended in the National Child and Youth Health Program (PNSIJ). To obtain health gains in this area, preventing the occurrence/recurrence of situations and avoiding danger, it is essential to invest in identifying children at risk and their surroundings and in training and raising awareness among nurses.
